Rehabilitative Care

Rehabilitative care is available to qualifying patients who have had an acute care stay at Grace Cottage Hospital, or patients transferring from acute care at other hospitals. Rehabilitative care eliminates the necessity of returning home prematurely.

Rehabilitative care is provided in an integrated manner with physician oversight of the total program. The primary goal is to help the patient return home at the highest level of independence possible.

Typical conditions include stroke, fractures and joint replacements, cardiac and pulmonary conditions, trauma, amputation and various neuromuscular or other debilitating conditions.

A medical provider can arrange for a rehabilitative admission at Grace Cottage Hospital by contacting social services.

Rehabilitative Services at Grace Cottage include, but are not limited to:

• 24 hour skilled nursing
• Home assessment performed by PT and/or OT in preparation for a patient’s return home
• IV therapy
• Laboratory and Blood Bank
• Medication adjustment
• Nutrition services including Total Parenteral and Enteral Nurtition
• Pain management
• Physical therapy, Occupation therapy, Speech & Swallowing therapy given 7 days a week
• Podiatry consultation
• Psychiatric consultation
• Radiology services
• Therapeutic recreation
• Tracheostomy care
• Wound and ostomy care
